Notice of Privacy Practices

A review draft about medical information, separate from website browsing. It is not yet the practice’s adopted HIPAA notice; coverage, procedures, and additional protections must be confirmed.

Patient rights

Where HIPAA applies, patients may request access, corrections, confidential communications, restrictions, an accounting of disclosures, and a paper notice. Limits and exceptions apply. The practice must confirm how to exercise these rights.

Uses, disclosures, and authorization

Applicable law can permit disclosures for treatment, payment, healthcare operations, and other specified purposes. Other uses require authorization. Psychotherapy notes and substance use disorder records may have additional protections, including restrictions on using Part 2 records in proceedings against a patient without the required consent or legal process.

Responsibilities and complaints

The adopted notice must explain safeguards, required breach notifications, privacy responsibilities, and complaint procedures without retaliation. The privacy official, verified request channels, and effective date remain pending. Ask the practice for its current notice; this draft does not replace intake documents.
