Good Faith Estimate Notice

Information for people who are uninsured or are not using insurance to pay for care. The practice’s applicable estimate procedures remain to be confirmed.

Requesting an estimate

For care subject to the federal rules, you can generally receive a written Good Faith Estimate if you request one or schedule services at least three business days ahead. Ask about expected charges before scheduling; delivery timing depends on the circumstances.

Keeping and comparing your estimate

Keep your estimate and bills. An estimate describes expected charges, rather than guaranteeing a final bill. A federal dispute process may be available if a provider’s bill is at least $400 above that provider’s estimate; generally, it must be started within 120 calendar days of the initial bill.

Contacting Caring Minds

Call or email the practice for general questions about estimates. The formal request and delivery process must be approved. This draft does not state fees, insurance acceptance, or payment requirements, and no estimate can be requested through a form on this site.
